Important Note Regarding Insurance

• If you have an HMO or a closed-network Medicare Advantage plan, our doctors will be considered out-of-network. Call your insurance and ask if they "allow out-of-network providers to place orders for labs, imaging, and referrals." If your insurance says they do not allow this, then reach out to us to discuss options.
• We are enrolled in Medicare as an "Opted-out" ordering physicians. This means that we have signed an agreement with Medicare that neither you nor I can submit our membership fees to Medicare for reimbursement. We are still allowed to submit orders for labs, imaging, medications, and referrals through Medicare for payment as usual.
• At this time, AHCCCS does not allow out-of-network providers to place orders.
