Banner Health

• Online Request: Use Banner's online record request system.
• Export through patient portal: Export your Medical Records from Banner Health Patient Portal
  1. Log In
     • Open your internet browser and go to https://mybanner.bannerhealth.com/sign-in.
     • Enter your username and password, then click Sign In.
  2. Find the Medical Chart to the left of the screen.
     • Click "Medical Chart."
     • Select "Continuity Documents" to see a list of visits.
  3. Download the File
     • Click on the visits you want to export.
     • Select "Download Codified" to download the file.
     • Save it to your computer.
     • File will download in .xml format
  4. Send or bring the file into the office. Options for sending:
     • Use our secure file upload tool.
     • Bring in on thumb drive or CD.
     • Send by emailing to info@arizonainternists.sprucecare.com (Our email server is secure for medical information, but yours is not. You must understand and agree to exposing information on to the internet on the way to our inbox)

• Additional Features: Manage documents, request records, and communicate with your care team through the portal.
